If you have not already enrolled for VA health care, please bring your DD Form 214 (Armed Forces Report of Transfer or Discharge), and a copy of both sides of your current insurance card (including Medicare or Medicaid) to our medical center. If you have questions or concerns at any time during your stay at the medical center, ask a member of your Patient Aligned Care Team, also known as PACT. Your PACT consists of your Primary Care Provider, RN Care Manager, LPN/Health-tech and a Medical Support Assistant. This teamlet is supported by a broader team which includes social workers, dieticians, pharmacists, mental health providers and case managers.
